How to get your online full GP record (prospective access)

From October 2023, patients are able to view their prospective full medical record. 

As a practice we have decided to ask patient’s permission first, as this gives the practice time to check the patient’s understanding and be sure access to their data is safe for them.

The practice is aware that not everyone will want access to their full record on the internet at this time, and some patients may have concerns about their personal medical records being made available on their smartphones or online, especially if other people have access to their devices.

  • If a patient does NOT want access, they will have the option to decline access.
  • If a patient DOES want access, they will be asked to opt in and complete an online access request form (which you can get from our reception) - so that their record can be individually reviewed by a clinician before online access is granted. Once reviewed and approved, the patient can view this online by logging into your account using the NHS app or NHS website.

Patients will need to provide proof of ID when they are handing in their request form.

There may be occasions when the data controller (GP partners) will withhold information kept in the health record, particularly if the disclosure of such information is likely to cause undue stress or harm to you or any other person.

Access to your full GP medical record

Online access to medical records is available prospectively, this means that you will have access to your medical record from the date that online access has been granted.

If a patient would like access to the full medical record, this is known as a Subject Access Request.  Patients will need to complete a Subject Access Request form (which is available from our reception).  The patient's full medical record is individually reviewed by a clinician before full access is granted.
