COVID PASS MEDICAL EXEMPTIONS - there are only very limited medical reasons why a patient cannot have the covid vaccination and only these patients will be eligible for a medical exemption pass. You MUST contact 119 to ask for an application form. You're GP practice does have access to the form. If you are eligible, the 119 service will send you a form.
